preface portfolio

ใกล้ถึงเวลายื่นพอร์ตแล้ว อยากให้พอร์ตเป็นภาษาอังกฤษแต่ไม่เก่งอิ้งสะงั้น 5555555 เลยอยากให้ช่วยแนะนำ grammar กับ tense ว่าถูกมั้ย
แต่งยังไงให้ดูเป็นทางการหน่อยสำหรับยื่นพอร์ตให้อาจาร์ยในคณะดู เราลองดูๆในเน็ตแล้วก็เอามาปรับเปลี่ยนนิดหน่อยๆ

"This portfolio is prepared for presenting about myself. It included my personal , education , activity and academic records. I accumulate file  contains documents about contributions that has been made , I was proud.
  I would like to use this opportunity to thank to my father , my mother , teacher and friends who provide assistance and advice to gather any significant evidence to document information. That is particularly useful to make it work this time , and to study in higher education."
