if she is a guy:

1. there is no way Maki won't know
2. When she slips, XXXXXX would have gotten the soft part of the guy's thingie hit...which should be quite painful.

Finally, XXXX gave Maki the candies. The only possible way for Aikawa to be able to keep the promise is if she is a girl:

If she is a girl, then she can act like a guy since they will never be sexually involved.
If she is a girl, then she can definitely appear as a girl to Saki.

However, Maki chose both...so we end up having this mixed ending...
