ใส่ที่อยู่ภาษาอังกฤษไม่ถูกครับ ใครรู้ช่วยแนะนำที

พอดีเราต้องการสมัคร บัตรเดบิตวีซ่าที่หนึ่ง แต่เรางงกับแบบฟอร์มที่ให้เรากรอกที่อยู่มาก ไม่รู้จะใส่ยังไง

เราเคยกรอกไปแล้วเหมือนกัน คือเราใส่บ้านเลขที่ตรง street number ตำบลกับอำเภอตรง street แต่ไม่ผ่าน 555..
แต่เค้าก็ตอบกลับมาและแนะนำว่า ...
