By July 9, it is transpired that investigators sifting through the wreckage of the doomed rocket had found
critical angular velocity sensors, DUS, installed upside down. Each of those sensors had an arrow that was suppose to point toward the top of the vehicle, however multiple sensors on the failed rocket were pointing downward instead. As a result, the flight control system was receiving wrong information about the position of the rocket and tried to "correct" it, causing the vehicle to swing wildly and, ultimately, crash.
มีเซ็นเซอร์หลายตัวถูกติดตั้งกลับหัว โดยที่บนเซ็นเซอร์เหล่านี้จะมีลูกศรบอกไว้ว่าให้ติดตั้งให้หัวลูกศรชี้ขึ้นบน แต่มีเซ็นเซอร์หลายตัวที่ติดตั้งโดยลูกศรชี้ลงล่าง ทำให้ระบบได้รับข้อมูลที่ผิดพลาดและพยายามปรับตำแหน่งของจรวดตามข้อมูลที่ผิดพลาด ผลก็เลยทำให้จรวดพุ่งไปสะเปะสะปะ และตกภายในเวลา 32 วินาที ห่างจากฐานปล่อยไปเพียง 1 กม. เศษเท่านั้น
น่าเสียดายว่าดาวเทียมกลอนาสจำนวน 3 ดวงก็พังไปด้วย ไม่งั้น GPS/มือถือ รุ่นใหม่ๆ ที่รองรับ Glonass ด้วยก็คงจะจับสัญญาณได้ดีขึ้นอีก
